Please notify us promptly of any change in your mailing address by downloading the form below. Please be sure the notice includes:

  1. Your signature or the signature of your appointed agent

  2. A valid telephone number and/or email address

  3. Includes your owner number

  4. Specifies your old address and new address, including ZIP codes

  5. Contains the last 4 digits of your Social Security number or tax identification number

Affidavit of Heirship

Affidavit of Heirship is a document that can be used in some states for the transfer of ownership of property left by a deceased person to their family. The Affidavit of Heirship can be used to transfer title from the decedent's name to the names of the heirs without going through often long court process.
